Before completing the Baptism Application below, please ensure you have the following information readily available:

An uploadable (pdf, jpeg, or png) file of the candidate’s birth certificate.
Contact details for both the mother and father, including their address, phone number, email, and Baptism information (if known).
The name and email address for each of the Godparents.

Please note that submission of the form is not possible without providing all required information, including the candidate’s birth certificate.
